Reservation Forms

Anyone who needs to use the sanctuary, Fellowship Hall, or one of the classrooms for a meeting or other function needs to complete a reservation form and submit it to the church office.

Print and complete the appropriate form and return it to the church office.  Please call 301-853-2224 if you have specific questions.

Classroom request form	Use this form if you need to reserve a room for a departmental/church-related meeting
Facility rental form	Use this form if you need to reserve a room for a private/non church-related function
Wedding package	Use this form if you would like to hold a wedding ceremony at the church
Rental rates	Rental rates for various church-provided amenities
